Initial Consultation

  • Includes 1 visit to your home

  • Cost: $200 for approximately one hour of your time

What to do:​

3.  Click the "Online Client Info Form" button below to submit electronically. If unable to do this, print out the "Client Info Form - PDF" (5 pages) below and complete prior to your visit if possible.

What to expect:

  1. Introduction and review paperwork

  2. Review paperwork

  3. Talk in depth about client's needs, goals, and safety concerns

  4. Appropriate referrals/resources will be distributed with coordinated communication if needed

  5. Discuss next steps of home safety assessment, including pricing, if needed

  6. Collect payment of $200 via credit card, check, or cash at completion of visit

Home Safety Assessment

  • Includes 2 visits to your home

  • Cost: $500 for the first floor plus $100 for each additional floor needing assessment, approximately 1.5 hours of your time

What to do:​

​1. Reserve a date and time slot that is available by clicking the "Schedule Services" button below, then click the "Book Now" button under Home Safety Assessment.

What to expect:

  1. Home Safety Assessment includes this visit plus an additional visit to discuss the home safety report

  2. Review client's needs and goals

  3. Home safety assessment performed 

  4. Appropriate referrals/resources will be distributed with coordinated communication if needed

  5. A comprehensive home safety report (example below) will be created that is tailored to the client's             unique needs with safety recommendations for each room that is assessed

  6. The assessor, client, and caregiver(s) will verbally agree on a home safety plan, with an emphasis on           high priority items

  7. Follow-up phone calls to the client and/or caregiver(s) will take place 1x/month for 6 months

  8. Collect payment via credit card, check, or cash at completion of visit

Follow-Up Visit

  • Includes 1 visit to your home, however, multiple follow-up visits can be scheduled as needed

  • Cost: $200 for each follow-up visit, approximately 1 hour of your time

What to expect:

  1. Review client's needs and goals

  2. Review any changes to the home or implementation of prior recommendations

  3. Re-assess the environment and provide additional home safety recommendations with pictures and purchasing links that will be sent to the client and/or caregiver(s) via email

  4. Provide any additional training needed for improved functional independence and safety in the home

  5. Schedule additional follow-up visits if needed

  6. Follow-up phone calls to the client and/or caregiver(s) will take place 1x/month for 6 months

  7. Collect payment of $200 via credit card, check, or cash

Virtual Home Safety Assessment

  • Virtual meeting for clients that are located outside of central PA

  • Cost: $300 for each follow-up visit, approximately 1.5 hour of your time

What to do:​

​Reserve a date and time slot that is available by clicking the "Schedule Service" button below, then click the "Book Now" button under Follow-Up Visit.

  2. Virtually tour property

  3.  Discuss safety hazards and/or concerns in the home

  4. A comprehensive list of home safety recommendations with pictures and purchasing links will be sent to the client and/or caregiver(s) via email

  5. Collect payment of $300, electronically
